Thank you for inquiring about employment with Emerald Medical Services. We do have open assignments across Delaware and all states. Listed below are the mandatory credentials needed to become an active employee with Emerald Medical Services. Included below are an application form, W4, I9, and a direct deposit form for you to fill out, sign, and then submit. - RESUME if not already on file with Emerald Medical Services.
- Healthcare Professional Delaware State license or certificate (such as RN, LPN, etc.)
- Driver’s License needed for a criminal background check/I9
- Social security card needed for a criminal background check/I9
- CPR/BLS, PALS or ACLS certification cards
- Physical exam or statement of good health record within one year including:
- TB – Yearly clearance results on any authorized documentation approved by Delaware Title 16 DHHS
- Immunization Records:
- COVID Vaccine Card – (or Medical/Religious Exception Statement)
- Hepatitis B – (3-part series with dates listed or an immunity titer) – (or ask Emerald Medical Services for a company declination form)
- Influenza (annually) – (or ask Emerald Medical Services for a company declination form)
- Selfie of shoulders and head for photo on Emerald Medical Services ID (professional looking).
- Malpractice insurance (While Emerald Medical Services does have insurance to cover employee, it is highly recommended that CLINICAL employees carry their own policy in addition. $1M individuals/$3M aggregate suggested). This is recommended but not required for CNAs.
- Complete Employee Application form, and the other forms attached below.
- Professional references from 2 previous supervisors must be listed on the application or on a separate form.
Once we receive all of these documents then:
- A criminal background check will be ordered. A Delaware BCC consent form must be signed, then, Emerald Medical Services will submit your name for fingerprints to the Delaware State police. Then a fingerprint form with your name on it will be sent to you separately.
- A drug screen at LabCorp will be ordered by Emerald Medical Services. Then a dated order letter will be emailed to you separately.
Once these items are in your Emerald Medical Services Personnel File, then a telephone orientation will be scheduled to go over all required documents.
